Qualifier X Diagnostic

Segundo as Guidelines da ICD-10-PCS o qualifier Diagnostic também se aplica nas drenagens e nós sabemos que estas se referem a fluidos e a gases.
 
Guideline B3.4a -  Biopsy procedures
Biopsy procedures are coded using the root operations Excision, Extraction, or Drainage and the qualifier Diagnostic.
Examples: Fine needle aspiration biopsy of fluid in the lung is coded to the root operation Drainage with the qualifier Diagnostic.
Biopsy of bone marrow is coded to the root operation Extraction with the qualifier Diagnostic.
Lymph node sampling for biopsy is coded to the root operation Excision with the qualifier Diagnostic.
 
Root Operation Drainage
Definition: Taking or letting out fluids and/or gases from a body part
Explanation: The qualifier DIAGNOSTIC is used to identify drainage procedures that are biopsies
Includes/Examples: Thoracentesis, incision and drainage
 
Como se pode ver a toracocentese é uma drenagem e pode ser diagnóstica.
Mas nem sempre foi assim e, talvez por isso, tenha surgido este mal entendido.
Até à versão de 2015 das Guidelines dizia-se:
 
Biopsy procedures are coded using the root operations Excision, Extraction, or Drainage and the qualifier Diagnostic. The qualifier Diagnostic is used only for biopsies.
 
A partir de 2016 desapareceu a especificação '... only for biopsies' e diz-se apenas:
 
Biopsy procedures are coded using the root operations Excision, Extraction, or Drainage and the qualifier Diagnostic
 
o que atesta que, mesmo na drenagem de líquidos, se pode aplicar o qualifier X Diagnostic.
 
É por isso que, consultando o Coding Clinic, encontramos os seguintes exemplos:
 
- punção lombar codificada como diagnóstica (CC 1Q 2014 p.9):
009U3ZX Drainage of spinal canal, percutaneous approach, diagnostic
 
- biópsia aspirativa por agulha fina do canal hepático comum codificada como diagnóstica (CC 1Q 2016 p.23):
0FB98ZX Excision of common bile duct, via natural or artificial opening endoscopic, diagnostic
 
- broncofibroscopia com "bronchoalveolar lavage (BAL), bronchial washings and cytology brushings were obtained in the right upper lobe bronchus; endobronchial biopsies and transbronchial biopsies from the right upper lobe; and bronchoalveolar lavage samples were obtained from the left lower lobe" codificada separadamente (CC 1Q 2016 p.26):

- lavagem broncoalveolar codificada como diagnóstica (CC 1Q 2017 p.51):
0B9J8ZX Drainage of left lower lung lobe, via natural or artificial opening endoscopic, diagnostic
 
- paracentese também codificável como diagnóstica quando especificada como tal (CC 3Q 2017 p.12)
 
e as seguintes explicações:
 
- "If the documentation does not specify whether the substance aspirated was tissue or fluid, assign the root operation Excision with the appropriate body part and the qualifier “Diagnostic.”" (CC 1Q 2016 p.24)
 
- "If there are two separate procedures, one diagnostic and the other therapeutic, then both procedures are code separately. For example, a diagnostic drainage procedure that uses a different approach or samples a different site from the therapeutic drainage procedure requires two separate codes to capture both the diagnostic procedure (biopsy) and the therapeutic procedure." (CC 3Q 2017 p.13)
 
Talvez esta outra instrução tenha contribuído para o equívoco:
 
- "If there is a therapeutic component to the procedure, only the qualifier “Z” is used, rather than the qualifier “X.” The qualifier “X” is exclusively used for diagnostic procedures only. If there are two separate procedures, one diagnostic and the other therapeutic, then both procedures are code separately. For example, a diagnostic drainage procedure that uses a different approach or samples a different site from the therapeutic drainage procedure requires two separate codes to capture both the diagnostic procedure (biopsy) and the therapeutic procedure."
 
Ou seja, quando um mesmo procedimento é terapêutico e diagnóstico (uma peça de mastectomia total que vai para anatomia patológica, uma paracentese evacuadora da qual também é enviada amostra para exame) só se codifica a terapêutica.
Mas se uma drenagem diagnóstica utilizar uma abordagem diferente da drenagem terapêutica, já há lugar à codificação dos dois procedimentos.
Do mesmo modo que, se uma biópsia for seguida de destruição, excisão ou ressecção, ambos os procedimentos (biópsia extemporânea e, por exemplo, mastectomia total) são codificados (Guideline B3.4b Biopsy followed by more definitive treatment) .
 
Em conclusão:
- não é o facto de se enviar uma peça para anatomia patológica que permite o uso do qualifier X Diagnostic;
- o qualifier X Diagnostic não se limita apenas a bipósias; a palavra chave, atualmente é Diagnostic.
- as punções lombares as toracocenteses e as paracenteses, mesmo que destinadas a bioquímica não deixam de ser diagnósticas; e não encontrei em lado nenhum (Guidelines, Coding Clinic) que Diagnostic se limite apenas a 'citologias'.
